Life and Health Insurance Claims - Critical Illness Life Insurance – Mortgage and Line of Credit

I’ve been diagnosed with an acute heart attack, cancer or stroke. I have Critical Illness coverage on my Mortgage or Line of Credit with TD Canada Trust. What do I do?

We will help you through the claims process making it easy for you every step of the way.

Step 1

Contact us to make a claim

Call our toll-free line at 1-888-983-7070 (Option1, 1, 1) to request the appropriate form from a customer service representative. If possible, have your policy number handy.

Step 2
Step 2:

Complete the forms

  • Get a TD Canada Trust branch representative to complete the form entitled “Claims for Group Creditor Critical Illness Insurance”.
  • Complete the “Claimant’s Statement for Group Creditor Critical Illness Insurance”.
  • Complete Section 1 of the “Attending Physician’s Statement” and have the attending physician complete Section 2.

Return the claim forms to us

Retain a copy of the forms and send the originals. You have 2 options on how to submit the forms:

  • Mail the forms to TD Life Insurance Company
    PO Box 1
    TD Centre
    Toronto, ON M5K 1A2 Canada. or,
  • Bring the original forms to a TD Canada Trust branch from where they will be sent by courier to TD Life.

Note: We will acknowledge receipt of your claim in writing.

Settlement process starts

In certain circumstances, during the settlement process, we may reach out to you or to your physician to request additional information.

Settlement is confirmed

We will contact you in writing to let you know if your claim has been approved and what the next steps are.
